The management of neuropathic pain from neuromas in the upper limb: surgical techniques and future directions

چکیده
The international association for the study of pain defines neuropathic pain as pain resulting from a lesion or disease in the central or peripheral nervous system.[1‐3] This categorization is broad and includes a range of etiologies such as trauma, lesions of the central nervous system, diabetic peripheral neuropathy, multiple sclerosis, and herpetic nerve lesions. When the upper limb is involved, it also includes chronic nerve compression, neuritis and complex regional pain syndrome. A significant proportion of pain in the upper limb results from neuroma formation. Currently, our knowledge of the underlying mechanisms is limited for pain resulting from complex regional pain syndrome and diabetic peripheral neuropathy. However, due to the wealth of research pain secondary to peripheral neuromas is reasonably well‐understood.

Unusual Complete Isolated Scaphoid Dislocation, Report of a Case
Isolated scaphoid dislocations are extremely rare injuries and are commonly associated with significant ligamentousdisruptions. A dorsiflexion-supination force upon the hand is considered as the most common mechanism of injury.Different treatment options have been proposed for the management of this uncommon entity, ranging from conservativetreatment with closed reduction and casting to a wide ...

متن کاملPediatric Traumatic Limb Amputation: The Principles of Management and Optimal Residual Limb Lengths
Pediatric traumatic limb amputations are rare and their acute and long term management can be challenging in this subgroup of patients. The lengthy and costly hospital stays, and resulting physical and psychological implications leads to significant morbidity. We present a summary of treatment principles and the evidence base supporting the management options for this entity. The initial manage...
متن کاملFreeze-thawed muscle grafting for painful cutaneous neuromas.
We used freeze-thawed muscle grafts to restore continuity to the affected nerve in 22 painful cutaneous neuromas. In 11 of the 15 neuromas in the upper limb, pain was partially or completely relieved; in six of these there was some recovery of distal sensation. Partial pain relief was achieved in only two of the seven neuromas in the lower limb. The difference is attributed to the longer grafts...
متن کاملA Rare Case of Triple Neuromas As a Cause of Chronic Stump Pain in an Upper Limb Amputee
Figure 1. During the exploration, large neuromas were identified to the ulnar, median, and superficial radial nerves. DESCRIPTION A 54-year-old man with through-wrist amputation of his right-dominant hand presented with chronic stump pain that was resistant to conservative treatment. Clinical examination was compatible with the presence of neuroma. Surgical exploration revealed neuromas of the ...
